What features distinguish polymorphous low grade adenocarcinoma (PLGA) and adenoid cystic carcinoma (ACC) with tubular pattern?
Ultimately, certain cytologic and histologic features:
 
PLGA - cells are polarized around a central tubular lumen, resembling a true gland.  Eosinophilic cytoplasm with rounded nuclear contours. Strong diffuse S100 staining.  Consistent EMA  but variable CEA staining.  Usually in minor salivary glands.
 
ACC - haphazard orientation of cells around a lumen.  Basophilic cytoplasm and angulated darkly staining nuclei.  Strong diffuse CD117 staining. Equally intense EMA and CEA staining.
 
The distinction is important because ACC is more clinically aggressive. PLGA and ACC have similar cytogenetic findings involving chromosome 12, e.g. t(6;12)(p21;q13).
